VT5 : Modified Sewing machine technique for closure of refractory cyclodialysis”.

A 15 year old male presented with severe hypotony (IOP 4mmHg) due to inferior 180 degree cyclodialysis. He had undergone a previous internal cyclopexy with endocapsular ring placed in sulcus; which failed to provide adequate internal tamponade as it was not supported in the sulcus by the zonules due to concurrent lenticular subluxation. Modified sewing machine technique was applied to zipper close the patent cyclodialysis cleft against the scleral rim. The 9’0 polypropelene suture was threaded into a 26G bent needle and the suture passed through the detached ciliary body in the sulcus apposing the ciliary body to the sclera. The auxiliary suture end was passed through successive loops and tied to the beginning end to zipper close the cleft. Post-operatively IOP in the child spiked to 24mmHg. To the best of our knowledge, this technique has never been described for internal cyclopexy in the past.
