A 23 year old male presented to the OPD with complaints of defective vision LE with involuntary movements of BE since childhood. Horizontal nystagmus with left eye esotropia alternating with right eye esotropia on cover test was noted. There was dampening of nystagmus on attempted adduction of the left eye. Visual acuity of right eye is 6/6 and left eye 6/18.Extra ocular movements was full in right eye and abduction was restricted in left eye.Anterior segment and fundus was normal in both eyes. On orthoptic evaluation left eye esotropia was 60 prism dioptre with abduction restriction. Exaggerated forced duction test was found to be positive in left eye. Diagnosis of nystagmus blockage syndrome was made. Bilateral medial rectus recession was done. Post operatively eyes were orthophoric with visual acuity of 6/6 in right eye and 6/12 in left eye. It suggests that even neglected cases when surgically corrected in early adulthood has favourable visual outcome in the amblyopic eye,
