Purpose: Compare outcome of trabeculectomy using Intraoperative or delayed postoperative MMC regime.
Methods: Randomized clinical trial on patients requiring surgery.
Group I- MMC 0.02% for 2 minutes during trabeculectomy.
Group II- Postoperative subconjunctival MMC injection (0.01 mg in 0.25ml volume) at 10-14 days.
Outcome measures: IOP, antiglaucoma medication (AGM), complications.
Results: There were 17 patients (Group I: 9; group II: 8). Both groups were similar in preoperative IOP and AGM. The median postoperative IOP at 1.5 year follow-up in Group I was 12 (after excluding hypotony cases, median: 17 mmHg) and in Group II was 15.5 mmHg which was not statistically different (p=0.41). The number of AGM required postoperatively in groups I (0.5±076) and group II (1±1.1) were comparable (p=0.49). Blebs in Group I were more avascular, 2 had persistant hypotony, 1 had persistant choroidals.
Conclusion: Postoperative MMC seems to be as effective, but safer than intraoperative use.
