Aim:Toanalyzethe incidence,risk factors,graft status&treatment modalities in patientswithpostpenetrating keratoplasty glaucoma. Methods :PostPenetratingkeratoplastyGlaucoma- elevated IOP (>21 mm Hg) following Penetrating Keratoplasty withorwithoutopticnervedamage& visual field changes. keratoplasties -performedover3years with 12 months follow up were retrospectively analyzed . Results-Secondaryglaucoma developed in 32of 120 eyes( incidenceof 26.7%). Corneal ulcer topped the list – incidence of post-PK glaucoma-43.8%;. Graft size of 8 mm and above there was increased incidence of secondary glaucoma(36%) . 71.8% -medical treatment, 28.1%-trabeculectomy with adjuvant Mitomycin C, of which 9.4% underwent combined procedure( phacoemulsification +trabeculectomy). Conclusion:. Inflammed ,infectedeyes& large grafts- moreprone to develop glaucoma . Majority of them responded to medical management. secondary glaucoma remains a challengefordiagnosisand treatment in post keratoplasty eyes
