a 21 year old female medical student came with sudden onset, nonprogressive diminution of vision in left eye after road traffic accident 1 week back.
general physical examination was normal.Vitals: normal.wound over left eyebrow at the junction of medial2/3 an1/3.
On ocular examination:Anterior segment:OS Normal.OD exotropia 7, RAPD.Vision OS6/6 OD 6/9. decreased colour saturation in OS noted.
Fundus:OU normal.visual field analysis showed superior arcuate scotoma in OS.CT head and orbit:linear fracture of left frontal bone, medial wall of left orbit with widening of medial extraconal compartment.MRI:fracture left frontal bone with extraconal hematoma with mild mass effect.
Diagnosis:traumatic optic neuropathy.
Treatment:injection Methyl prednisolone 30mg/kg for 3 days followed by oral prednisolone in tapering dose.
Result:Vision recovered to normal.
Conclusion:Early diagnosis and timely intervention in traumatic optic neuropathy has a better visual outcome.
