We describe an unreported manifestation of Takayasu arteritis with secondary nasolacrimal duct obstruction due to fungal granuloma. Takayasu arteritis has been reported to predispose patients to systemic secondary fungal infections due to intrinsic immunologic abnormality and immunosuppression due to long term steroid therapy. This is the first case report of the pathology in the periocular region in a 19 year old boy having Takayasu arteritis . Fungal granuloma was suspected on imaging and confirmed on histopathology of the medial canthal mass which caused nasolacrimal duct obstruction . Most common ocular manifestations of Takayasu arteritis are ocular ischemic syndrome and proliferative retinopathy , which in our case , were ruled out by Retina expert .Takayasu arteritis patients may first develop fungal infection in the periocular region. Prompt diagnosis by ophthalmologist can alert the physician about secondary fungal invasion and improve overall prognosis
