A 72 year old male presented to us with gradual painless proptosis of the left eye since last 4 months. No associated systemic symptoms. O/E: LE showed 3 mm of eccentric proptosis with evidence of pink vascular firm mass stuck on superotemporal aspect of eyeball. CT scan shows fairly defined homogeneous mass along lateral orbital margin, from conjunctival surface of eyeball extending posteriorly to orbital apex, engulfing the lateral rectus muscle. He underwent diagnostic incision biopsy. Histopathology showed smooth muscle malignant tumor and immunohistochemistry analysis confirmed the diagnosis of leiomyosarcoma. FDG PET scan ruled out involvement of other organs. Diagnosis of primary orbital leiomyosarcoma was made. He underwent eyelid sparing total orbital exenteration of the Left eye. External beam radiation to left orbit is planned after 6 weeks. Primary orbital leiomyosarcoma is rare in adult male. We report a histopathologically proven case of the same.
