A prospective study was carried out in 30 patients. Group 1 was 15 patients who underwent unilateral silicone sling surgery and group 2, 15 patients who underwent bilateral fascia lata frontalis sling with contralateral levator excision. Post operatively lid height, symmetry, lid crease were studied. Post operative MRD1 was 2.91+1.10 in group 1 and 3.8+0.52 in group 2 (p value 0.008). Post operative symmetry in group 1 was excellent in 11 (73.3%), good in 3 (20%) and poor in 1(7.7%). In group 2, symmetry was excellent in 14 (93.3%) and good in 1 (6.7%). Excellent lid crease was seen in 12 patients (80%) in group 1 and in all patients in group 2. The use of fascia lata for unilateral severe congenital ptosis resulted in higher functional rates as compared to silicone sling.
