FP683 : Posterior hump for ultrasonic differentiation in cases with diffuse choroidal excavation

We aimed to use ultrasound for differentiation between myopic posterior staphyloma (MPS) and choroidal coloboma (CC) in this pilot prospective cross sectional case series. 27 eyes of 20 patients undergoing cataract surgery with choroidal excavation on ultrasound were included. None of the patients had an iris coloboma. This was a triple masked study and the sonographer, examiner and image reader were masked from each other findings. A posterior hump was detected at the edge of the excavation in all the eyes with CC, while only 13% of the eyes with MPS were found to have it. This sign had high sensitivity (100%), specificity (87%), positive predictive value (86%) and negative predictive value (100%) for detection of CC. Therefore we concluded that a posterior hump at the transition of normal to ectatic region upon ultrasound of a diffuse choroidal excavation is indicative of CC in patients with insufficient optical media.
