Angiostrongylus cantonensis, which, to the best of our knowledge, is the third case report from India and first case from Madhya Pradesh.

Seventy years old female presented with complaints of watering, redness and on-off foreign body sensation in right eye since 2 months. On examination best corrected visual acuity in right eye was 20/200 and left eye it was 20/120. Right eye showed mild conjunctival congestion and corneal haze. On slit lamp evaluation, a translucent worm of approximately 15 mm length was found freely moving in anterior chamber. Both the eyes had nuclear sclerosis grade II. Undilated fundus examination of both eyes was normal. All routine investigations were normal.

Patient underwent surgical removal of live worm under topical anaesthesia. It was confirmed to be Angiostrongylus cantonensis by microbiologist. At post operative one week, patient was symptomatically better, her corneal haze disappeared. Now, cataract surgery is planned for the patient.
