Dr.Rakhi Mehta, M08055
SYNOPSIS A 35 year old young man(examined elsewhere)complained to have history of a faint grey round spot in front of left eye. Vn (OD) 6/6, (OS)6/9P. Fundus – orange red nodular subretinal spots. FFA and OCT showed serous detachment near macula; was treated as CSR (elsewhere). A month later he was examined by us in our centre .FFA, ICG and OCTAngio showed no CSR but IPCV with BVN.OCTA – shallow SRF, NOTCH between polyp and the adjacent PED and DOUBLE LAYERED sign due to hyperreflectivity of choroidalayers.Patient given Intravit inj.Eylea(anti VEGFagent afibercept)improving vision to 6/6 P.Patient is on follow up likely to show more improvement after next injection. IPCV- variant of neovascular AMD; branchingvascularnetwork BVN with polypoidal dilated vessels. On ICG- nodular hyperfluoresence with hypofluorescent halo.Better diagnostic techniques help in early detection of IPCV; aid in treatment with PDT/Laser photocoag /anti VEGF depending upon location and leakage from polyp and BVN.
