A patient with pseudophakic bullous keratopathy underwent descemet stripping endothelial keratoplasty. Postoperatively graft was attached and recipient’s corneal edema resolving. Patient was non-compliant and stopped using steroids. 3 months later he presented with cloudy fluid collection measuring 4mm inferiorly between the graft-host interface. Edges of graft were completely attached with a quiet anterior chamber and no other clinical signs of acute rejection/infection. Interface fluid drainage was done by a venting incision in dependent part of cornea. No microorganism growth was observed on culture of fluid. Patient was started on hourly steroids.One-month post drainage graft remained attached and recipient cornea compact
Conclusion-Fluid in the graft host interface post DSEK is reported in immediate post-operative period, but its appearance 3 months post-surgery in a previously attached graft is unusual. Since no organism was isolated, the source of fluid may be from stromal melt
