Introduction: Purtscher retinopathy is bilateral peripapillary patches of retinal whitening and haemorrhage caused by microvascular occlusion in severe trauma. Purtscher-like retinopathy is seen in cases other than trauma. Case report: An 18-year old male with carcinoma transverse colon presented on postoperative day 3 following hemicolectomy with sudden onset, bilateral, painless diminution of vision. On examination, visual acuity was counting fingers at 1 metre in both eyes. Colour vision was normal. Amsler’s grid showed metamorphopsia in both eyes. Anterior segment was within normal limits with normal pupillary reactions. Fundus showed multiple patches of retinal whitening over the posterior pole, with a predominantly peripapillary distribution, with macular edema in both eyes. OCT macula showed intraretinal edema and mild subretinal fluid in both eyes. Conclusion: Purtscher-like retinopathy could rarely be caused by carcinoma colon.
