Purtschner like retinopathy is rare cause of postpartum visual disturbances. Visual recovery in such patients varies vastly and till date there is no definitive line of treatment to be followed. We report a case of 24yrs old primigravida who developed sudden onset bilateral diminution of vision within 24 hrs of vaginal delivery of stillbirth child at full term. IOP was normal. There was positive history for preeclampsia and negative history of HELLP Syndrome, HUS and Connective tissue disorders. Fundus photo showed characteristic multiple bilateral purtschner fleckens, superficial hemorrhages, disc swelling and macular edema, which was confirmed by OCT. No intervention was was done. Patient had gradual recovery of visual functions with some visual acuity impairment. Though the exact pathogenesis is unknown but in this case coagulopathy appears to be the cause, as supported by significantly increased D-dimer value.
