20 year old male came with blurred vision in right eye since 3 days and trauma history to left eye with stick 1 month back which was conservatively treated. Right eye showed mild anterior uveitis, 6/6 vision with normal fundus and left eye showed presence of 4mm sealed corneal tear plugged with iris in inferotemporal cornea with distorted pupil & pigments over lens. Diagnosis of sympathetic ophthalmitis was made and treated with high dose of prednisolone 1 gm /day and topical steroids tapered over 2 months. Decision was taken not to enucleate the left eye as sensitisation had already developed. Usually sympathetic ophthalmia presents as bilateral diffuse uveitis with granulomatous anterior chamber reaction. However the inflammation can be mild in which the term sympathetic irritation is used. Therefore timely referral of such penetrating injury patient for repair is important and it should be remembered that diverse clinical presentations are possible with SO as seen with above case.
