Toxoplasmosis is the most common opportunistic infection in patients with the acquired immunodeficiency syndrome (AIDS). It presents as potentially blinding necrotizing retinitis with a progressive and relapsing course when CD4+ T counts falls below 250/mm. This unique case which was never reported in the past, highlights the rare presentation of Ocular toxoplasmosis in HIV individual at higher CD4+ T counts in a 42 year old man .Anterior segment examination showed anterior chamber cells and fundus evaluation – vitreous cells with a dense opaque retinitis patch involving inferior half of fovea. He was seropositive for HIV and on HAART medication for 10years. Investigations revealed positive Toxoplasma gondii serology (IgG+/IgM+), positive mantoux, CD4+ T counts was 777 cells/mm. He was treated with Trimethoprim/ sulfamethoxazole, azithromycin and oral steroids after 48 hours. On follow up there was resolution of retinitis and improvement in visual acuity .
