We report a case of intraconal grease gun injury accompanied by traumatic optic neuropathy in a 20-year-old male.He presented with dimness of vision,chemosis,proptosis and restricted extraocular movement (EOM) in the left eye(LE)following an accidental injury.Visual acuity in LE was FC1 metre,pupil was widely dilated and non-reacting.An yellowish white thick discharge(grease)was continually exuding from the inferior fornix of the LE.Orbital computerized tomography(non contrast)revealed a crescent shaped hypodense area in the medial intraconal space.He was started on pulse steroid therapy along with intravenous ceftriaxone.But as the condition detoriorated, urgical exploration of the orbit was planned on day 5.Visual acuity ,chemosis ,proptosis and EOM improved rapidly after surgical removal of the grease by anterior orbitotomy.Grease gun injuries to the orbit have rarely been reported.This case is the eighth report throughout the world,and the first in India to our knowledge.
