This was a retrospective comparative study of 60 consecutive eyes. Patients with idiopathic macular hole size measuring more than 400 microns and minimum follow up of 6 months were included in this study. Group A underwent internal limiting membrane peeling alone and Group B underwent Inverted internal limiting membrane flap surgery, both with or without combined phacoemulsification with intraocular lens implantation. The study is designed to compare anatomical and functional outcomes as Preoperative and postoperative BCVA (Best corrected visual acquity) ,macular hole size, macular hole form factor and macular hole index using swept source OCT(optical coherence tomography). In conclusion, inverted ILM flap technique appeared to be a safe and successful approach for the management of large idiopathic MHs with promising anatomical and functional results.
