PURPOSE:To compare limbal relaxing incisions (LRIs) and clear corneal incision on steepest meridian (ISM) in phacoemulsification surgery to reduce pre-existing corneal astigmatism.

METHODS:This prospective study included patients having ≤ 3 Diopters(D) of corneal astigmatism who were randomly assigned to 2 surgical techniques:Phacoemulsification with a single clear corneal incision placed on the steepest meridian or LRIs consisting of paired incisions straddling the steepest corneal meridian along with phaco incision.Keratometric astigmatism evaluation done 6 weeks postoperatively.

RESULTS:Sixty eyes of 60 patients were evaluated,30 in the ISM group and 30 in LRI group.Six weeks postoperatively, the flattening effect was 0.43 D (range 0.18 to 0.77 D) in the ISM group and 1.22 D (range 0.45 to 2.24 D) in LRI group (P=0.002) which was significant.

CONCLUSION:LRIs were more effective than incision on steep meridian in the correction of mild to moderate pre-existing corneal astigmatism.
