Purpose: To evaluate morphologic changes in Oct in the eyes undergoing vitrectomy with & without adjunctive use of Anti VEGF after vitrectomy and to see if visual acuity improvement was different in the two groups.

Methods Prospective randomised study of 217 eyes with PDR. Eyes with tractional retinal detachment at the macula-observed preop or intraop were excluded. Group I (107 eyes) pre op anti VEGF Group II (110 eyes) no anti vegf preop.

Results: Pre-op VA groupI – 1.27 logMAR & GroupII – 1.21 logMAR. PostopVA group I- 0.26 logMAR & group II – 0.30 logMAR. difference being statistically significant (p<0.05). The external limiting membrane disrupted -17% eyes-groupI & 22%-groupII. Ellipsoid zone abnormality(EZA) 18.4%-group I, 23.7%-groupII. Eyes with EZA had statistically poorer visual acuity- both groups. This study shows use of Anti vegf as a preoperative adjunct helped restore macular function enabling better visual results
