Occlusion of retinal vasculature is quite rare;seldom reported in verteporfin treated circumscribed choroidal hemangioma(CCH)
Asymptomatic peripapillary CCH (4DD)for past 5 years with sudden onset defective vision. Exudative retinal detachment involving fovea clearly explained the defective vision.5year slow growth in the tumour size from 18.29mm2 to 27.34mm2. Multimodal imaging established the CCH
As there were clear indications, subject underwent FF PDT at a distance of 200 um from the optic disc edge. Overlapping 2 spots of GLD 5245u were applied to cover the entire surface of the tumour. Striking feature was the presence of branch artery occlusion over the tumour which was confirmed by the eye tracking OCT. Followup showed complete tumour regression with meaningful improvement in BCVA.
Occlusion of retinal vasculature in PDT occur if hemangioma is peripapillary as the photosensitised area is closer to optic disc. PDT is effective;taken with care in lesions close to disc.
