126 cases of cataract with grade 4 + nucleus were operated between May 2016 and March 2018 with this new technique. All cases were video recorded intraoperatevely and on first post operative day to access clarity of cornea . In all cases flared 2.8 mm phaco tip was used. To drill bore in centre of nucleus 70% power in contentious mode and 100 mm vacuum was used . To chop the nucleus blunt chopper with inner sharp edge was used for vertical chop in the capsular bag .For emulsification of pieces Phaco setting were vacuum 400 mm and power 50-60% in pulsed mode of 24 pulses was used. Emulsification of pieces was carried out in the bag or pupilary plane.
11 patents had mild Striate keratitis which was cleared within 3 days, 115 patients had crystal clear cornea on second post op day. One patient had PCR.
From the results we can conclude that this new technique of Phacoemulsification is safe in very hard cataracts and maintains clear cornea on 1st postoperative day.
