Foster Kennedy syndrome describes the clinical syndrome of unilateral optic atrophy with contralateral papilledema caused by a compressive mass lesion often in anterior cranial fossa. This is a case report of a 15 year old boy presenting with progressive right facial distortion, eccentric proptosis of right eye since last 10 years with diminution of vision, diplopia and anosmia. The physical examination showed increased intercanthal distance, mass in nasal cavity pressing over hard palate, visual acuity of 6/60 in right eye, 6/24 in left, hyperaemic disc in right fundus and disc pallor in left. CEMRI brain revealed an ill-defined,multiloculated solid cystic lesion involving frontal,sphenoid,maxillary and ethmoid sinuses with architectural distortion and intracranial extension. Right partial maxillectomy and excision biopsy was performed. The histological report showed cellular meningothelial neoplasm with Psammoma bodies suggesting a mixed variant meningioma(WHO grade 1).
