We report the case of 52 years old male presenting with decreased visual acuity on her left eye and ipsilateral headache and protrusion of left eye ball. On examination we found proptosis, raised IOP. Systemic work up done. On CT scan we found extraocular muscle enlargement and dilated superior ophthalmic vein confirming diagnosis of carotid cavernous fistula. Though digital subtraction angiography is gold standard technique for diagnosing carotid cavernous fistula but CT is the most accessible modality in almost every settings. Carotid cavernous fistula is a rare clinical entity which will often present with ocular manifestations which can ultimately lead to full vision loss. prompt diagnosis and management can allow for potentially vision and life-saving interventions.
