A 24 year female presented to opd for Headache off & on since 2months.
No other associated complains
On exam: vision Both Eyes – 6/24 BCVA
Color vision -WNL
Anterior segment Both Eyes – WNL
IOP –BE -16 & 18
BOTH EYES-Marked Degree of DISC OEDEMA With Flame Shaped Haemorrhages ,Cotton Wool Spots & Hard Exudates.
Macular star – Left eye .
This fundus picture – Grade 4 Hypertensive retinopathy.
Lab test
KFT: deranged , urea – 89mg/dl(20-45),
creatinine-9mg/dl(0.6-1.2).
While other test –LFT ,Coagulation tests & serum electrolytes –WNL
ECG –WNL
Renal USG-Small kidneys than normal ,with loss of corticomedullary diffentation & increased echogenicity .
Diagnosis –Chronic Renal Failure was made
Patient was started on dialysis twice a week .
Visual acuity of patient improved to 6/9 both eyes after normalisation of blood pressure.
Now she is planned for Kidney Transplantation.
