A 28 day old baby was admitted in the intensive care unit and was diagnosed to have congenital hear disease. Ophthalmological opinion was sought for the abnormal appearance of bilateral upper lids noticed by the internist. On examination , the child was found to have features typical of eyelid imbrication syndrome. Eyelid imbrication is an extremely rare pathology with only 5 cases in literature and 3 cases of Indian origin. Thorough ophthalmological examination revealed no ophthalmological predisposing cause for the development of the lid condition. Pathogenesis of eyelid imbrication is unknown. A similar entity is seen in adults known as floppy eyelid syndrome . histopathological studies have implicated presence of elastase as a causative factor for FES. Role of elastase has also been establishes in congenital heart disease. We propose the role of elastase as a causative factor for both eyelid imbrication and CHD in this child
