Purpose-To describe diagnostic & management difficulties in keratoconic eyes with pachymetry too
low for collagen cross linking(CXL) & yet significant improvement in vision with rigid contact
lens(RCL) that immediate keratoplasty can be deferred.
Methods-Retrospective case series of 18 eyes.Inclusion criteria-keratoconic eyes with pachymetry
too low for CXL with significant improvement in vision with RCL.Exclusion criteria-Keratoconic eyes
with a)adequate pachymetry for CXL b)without significant improvement in vision with RCL c)with
hydrops.Results-Mean age-25.2years.Mean BCVA with glasses-0.48logMar.Mean BCVA with RCL-
0.17logMar.Mean steep k -63.53D.Mean central pachymetry-302.5µ.Mean of highest posterior
surface elevation-103.5µm.Mean follow up-5 years
Conclusion-Quantitative analysis of progression of keratoconus in these eyes is difficult as imaging
techniques become unrecordable in advance stages.Significant improvement in vision with RCL
makes the decision to operate difficult.
