FP294 : Cilioretinal artery occlusion : a rare complication of photodynamic therapy.

CASE: A 65 years female, known case of polypoidal choroidal vasculopathy was treated with combination therapy – full fluence photodynamic therapy (PDT) and intravitreal Anti-VEGF injection. The large spot size of 5450mm was close to the disc. After 1week, patient had drop in vision from 6/12 to counting finger 1meter. Optical coherence tomography (OCT) showed hyperreflectivity of inner retinal layers, large serous macular detachment (SMD) and pre-existing Pigment epithelial detachment (PED). Initial diagnosis of reactive macular edema was made and treated with intravitreal triamcinolone. After a month, SMD resolved with persistence of inner layer hyperreflectivity. Color photograph showed sclerosed cilioretinal artery, confirming the diagnosis of cilioretinal artery occlusion. As per our knowledge this is the first case reporting cilioretinal artery occlusion post PDT.

CONCLUSION: In cases having cilioretinal artery , large spot size of PDT close to the disc should be avoided.
