Tumours of lacrimal Gland are rare in clinical practice. Among all of them, the most common epithelial tumour is Pleomorphic Adenoma of Lacrimal Gland. Here we are presenting a case of ploemorphic adenoma of lacrimal gland with intraconal extension, which is a rare occurrence. A 45 year old female presented with painless, progressive, eccentric proptosis of left eye for last 1 year. Contrast Enhanced MRI brain with B/L Orbit showed multilobulated enhancing intraconal mass in left orbit. Mass was surgically removed by anterior orbital approach with lateral extension. Histopathological examination revealed Pleomorphic Adenoma of lacrimal Gland. In the post operative followup there is significant improvement in proptosis and dystopia with minimal residual ocular motility defect. Although intraconal extension of Pleomorphic Adenoma of Lacrimal Gland is rare, it should be considered as one of the differential diagnosis for any intaconal mass presenting with proptosis and dystopia.
