This study aimed to compare aspherical toric intraocular lens (T-IOL) (study group) implanted bilaterally with monofocal IOL (control group) in patients with cataract and corneal astigmatism (CA). A prospective, hospital based study was undertaken in a tertiary care hospital from April 2016-March 2017. 84 cases with bilateral cataract and CA of at least 1.25 D were randomized to receive either bilateral toric (40) or monofocal (44) IOL implantation. Main outcome measures were uncorrected distance visual acuity (UDVA), residual astigmatism(RA); assessed at one and 6 months intervals. Corneal astigmatism was 1.75 (0.95) D and 1.70 (0.84) D in toric and control groups, respectively. Postoperative UDVA of 6/6 was 82.5% in toric group compared with 36.4% in control group (P < .001). Spectacle independence for distance vision was achieved in 87.5% vs 31.8% and mean RA was -0.65 D and -1.75 D in the two groups. Thus unaided distance visual acuity was significantly better in toric group.
