Orbital retinoblastoma is an advanced form of retinoblastoma seen mostly in developing countries with the incidence of 18–40% .Primary orbital retinoblastoma is the orbital ,extension of the disease where as Secondary orbital retinoblastoma occurs in an enucleated socket after an uncomplicated surgery . We describe a case of 6 year old boy came to us with the complaints of pain and swelling of the left orbit for 15 days, which was enucleated 6 month ago elsewhere. The patient was diagnosed as a case of retinoblastoma and enucleation was done six month ago.After six month he devoloped large swelling in the left orbit with restriction of the ocular movements. . On contrast enhanced CT scan orbit showed soft tissue mass lesion approx 5.6 cm x 4.7 cm x 4 cm . 3 cycles of neo adjuvant chemotherapy was given to the patient.Patient was planned for surgical excision of the residual mass after 6 cycles of chemotherapy.
