Children who underwent cataract surgery for congenital cataract without IOL implantation before 1 year of age were included. All children underwent detailed examination under anaesthesia including biometry, intraocular pressure and fundus examination. Ultrasound Biomicroscopy (UBM) was done to assess for adequacy of sulcus support. A total of 30 eyes of 18 patients were included. The mean age was 66+/-14.1 months. Adequate ciliary sulcus was noted in 36.6% cases on slit lamp examination and 96.6% with UBM. At secondary implantation, 40% underwent IOL in sulcus without optic capture, 56.6% in sulcus with optic capture and 3.3% scleral fixated IOL. IOL were noted to well centered and stable on postoperative UBM(tilt <5°) in 96.6%. The rate of VAO was 3.3% & secondary glaucoma was 10%. The median visual acuity at final follow-up was 20/60. The secondary IOL implantation in children with UBM assist can guide in better surgical planning, minimal manipulation with good visual outcomes.
