FP1864 : Implantable Collamer Lens in a missed Retinopathy of prematurity

A 23 year old girl with -13DS OU underwent Implantable Collamer Lens(ICL) implantation at a private hospital. She presented with acute loss of vision OD after 6 months with best corrected visual acuity (BCVA) OD 5/200 and OS 20/120. Both eyes had ICL with very shallow vault and anterior subcapsular cataract OS>>OD. Fundus OD showed total retinal detachment(RD), superotemporal tear with peripheral avascular retina (zone 3) and OS peripheral avascular retina (zone 3). Mother of patient gave history of prematurity (28 weeks of gestation). Patient underwent OD buckling surgery and OS pIOL explant with cataract surgery followed by prophylactic laser of avascular retina. On follow up OD retina remained attached at 1year with BCVA 20/80; OS BCVA 20/40 with retina well lasered. Hence, peripheral sceening is of utmost importance before taking patient for ICL surgery. ICL are best avoided in ROP patients due to crowded anterior chamber and retinal changes which increase risk of cataract and RD.
