FP1860 : PERIOCULAR SEBACEOUS CELL CARCINOMA WITH ORBITAL EXTENSION

PURPOSE-Sebaceous carcinoma is a rare,malignant,lethal tumour occurring most commonly in upper eyelid due to greater number of meibomian glands.

METHODS-A 65year old male presented to ophthalmology OPD with mass in left upper lid since 2 years.Visual acquity was 20/40p and 20/125 In right and left eye respectively.Pathological analysis revealed sebaceous carcinoma.MRI of brain and neck shows the tumour abutting the lacrimal gland and orbit with loss of fat planes.A conservative surgery was planned. The patient has been informed about the dissemination.

RESULTS-Wide local excision with upper lid reconstruction with tarsorraphy based forehead flap was done.Pathological analysis revealed clear resection margins on all samples.The pathological stage was pT2MxNx.

CONCLUSION- Wide local excision for low grade and exenteration for invasive lesion are surgical options.Sebaceous carcinoma can recur in 33% of cases.Despite complete tumour resection, long term followup is mandatory in patients.
