A 38 year old male with acute lymphocytic leukemia (ALL) undergoing chemotherapy now in remission phase came with diminution of vision in both eyes since 3 days, was found to have bilateral leukemic optic nerve head infiltration and no other neurological involvement. He was managed with Methylprednisolone pulse therapy and intrathecal methotrexate following which there was slight improvement in vision. It is stressed that any ocular symptoms in ALL should be investigated without delay by an Ophthalmologist as optic disc infiltration is considered Central nervous system involvement and needs prompt treatment.
