Aim: To report a case of toxic keratitis probably secondary to topical podophyllum application for eyelid warts. Design: Case report, interventional. Methods: The authors present a 32 years old male with acute onset corneal subepithelial haze with sterile infiltrates within 24 hours of application of topical podophyllum. Vision at presentation was 6/9 which deteriorated to 6/18 in 2 days. Topical steroids along with cycloplegics and lubricants were used in the treatment. Results: The haze gradually cleared up and almost disappeared with minimal scarring and BCVA 6/6 by 2 months with the use of topical steroids. A score of 6 was obtained using Naranjo’s adverse drug reaction probability scale. Conclusion: Topical podophyllum can be a probable cause of toxic keratopathy resulting in diminution of vision.
