64 year male who presented with diminished vision in OU since 1 year with history of intermittent episodes of redness.OU BCVA was 6/18.Slit lamp exam-OU cells +2,anterior chamber flare, PPM with branching iris pattern,posterior synechiae,nuclear sclerosis grade 2-3with anterior subcapsular cataract suggestive of chronic iridocyclitis.OU fundus normal. Investigations showed ANA positive,raised serum complement levels and HLA-B27 negative. OD cataract surgery by clear corneal phacoemulsification with pupilloplasty with intraocular lens implantation done under cover of perioperative systemic and topical steroids.There was minimal bleeding intra-operatively.3 weeks later,eye was quiet with BCVA6/9.PPM can lead to amblyopia in children,visual disturbances in adults & presents a challenge during cataract surgery. This case highlights importance of judicious use of perioperative steroids & prudent surgical management in such cases for minimal postoperative inflammation & good visual outcome.
