A 53 years old monocular female patient was referred to the contact lens clinic for visual rehabilitation .She had a history of corneal transplant done in the left eye for keratoconus. The best corrected visual acuity in the left eye with glasses failed to improve beyond 6/18 N18 due to irregular astigmatism in the corneal graft. The corneal graft was clear centrally but ectasia could be appreciated inferonasally close to the graft host junction. Corneal topography confirmed severe ectasia at around 7o’clock position with surrounding flattening.The complex geometry of the graft was the main challenge to contact lens fitting in this patient. A specialized semi-scleral lens, RoseK2 XL (Diameter 14.6mm, Base Curve 6mm) was chosen for trial based on topography. An acceptable fit was achieved as confirmed by slit lamp examination and Anterior Segment Optical Coherence Tomography (AS-OCT). The visual acuity of the patient improved to 6/9 and the contrast sensitivity to 1.5 Log units.
