A 19yr and 17yr old male came with complaints of drooping of eyelid,inability to move the eyes,outward deviation of OU since childhood.Visual acuity was OU 6/24 in the elder brother and OU 6/18 in the younger.On further examination OD mild ptosis and OS moderate ptosis was seen in both cases with OD exotropia of ~30-40⁰ and OD hypotropia~10-15⁰.Ductions and versions were restricted severely in upgaze bilaterally. Aberrant eye movements were absent.Force Duction Test was positive for restriction.Both patients were born out of non-consanguineous marriage and parents were absolutely normal which suggested AR inheritance.Based on their clinical presentation, diagnosis of congenital fibrosis of extraocular muscles was made and further confirmation by genetic typing is awaited.Case is still under evaluation and has been proposed to undergo transposition of vertical recti with suture augmentation with lateral rectus recession at maximum and ptosis correction.
