Anterior plageocephaly with receding forehead on one side due to synostosis of coronal sutures can produce underaction of superior oblique(SO) due to desagitalisation of the tendon which adds to existing asymmetry.Correction of ocular torticollis can improve the facial asymmetry if surgery is undertaken before 2 to 3 years.A retrospective long term analysis of 6 children operated forSOparesis due to plageocephaly was done.All had canthus to angle of mouth measured for facial asymmetry before surgery and during follow up.Photos taken. 4girls and 2boys. 2 were sisters.4 had right plageocephaly and 2 left.4 had inferior oblique recession and 2 had inferior oblique anterior transposition done.Preop PBCT showed mean angle of deviation 18 pd of hypertropia and postop mean deviation was 4pd hypertropia. The follow up showed improvement of asymmetry as seen by reduction in lateral canthus to angle of mouth distance from mean 2.6mm to 0.8 mm and photos at the end of 2 yrs.So early surgery .
