Objective: To find correlation of severity of diabetic retinopathy (DR) with dry eye parameters.

Method: One hundred and twenty middle aged patients (120 eyes) with diabetes were included in the study between January and December 2017; consecutive sixty with DR and sixty without DR. Besides biochemical parameters; Schirmer 1 & 2, tear film break up time (TBUT) were estimated in all cases.

Result: Age (p=0.014) and HbA1c (p=0.021) were significantly more among DR group. Schirmer 1 (p=0.001), Schirmer 2 (p=0.001) and TBUT (p=0.007) were significantly lower among DR. This difference persisted after adjustment for age and HbA1c. Risk of DR increases with low TBUT (<10.5,OR 4.76); Schirmer 1 (<12.5,OR 6.1) and Schirmer 2 (<9.5,OR 8.09). Severity of DR was correlated with Schirmer 1 (p=0.02), Schirmer 2 (p=0.048) and TBUT (p=0.044). No correlation was observed between dry eye parameters and HbA1c in either group. Conclusion: Dry eye parameters bear positive correlation with severity of DR.
