Retrospective observational study was done in 150 eyes that underwent optical PK with 1 year follow up. We used multiple tests to find out association between risk factors and graft failure, compare median survival of graft, identify independent risk factors for predicting graft failure.Mean age was 53.76±16.51years. Bullous keratopathy & degenerations 52% was leading indication for surgery followed by adherent leucoma 27.30%, corneal opacity 9.32%, dystrophies 5.99% and corneal scars 3.33%. Preoperatively 56.66% patients had PL PR vision. Visual outcome was 41.33%, 13.33% & 45.33% good, moderate & no improvement respectively. Graft failure in 46%. Median survival of grafts was 26 months (95%). Corneal vascularization, age(P=0.007),immunological rejection(P=0.0001), postoperative glaucoma(P=0.0007) were statistically significant.Corneal vascularization, age, immunological rejection, graft infections and postoperative glaucoma were identified as significant risk factors for graft failure.
