PURPOSE Report a case of spontaneous bilateral late dislocation of in‑the‑bag intraocular lens in uveitis patient. METHOD A 57-year-old male with bilateral uveitis underwent uneventful phacoemulsification in both eyes. Six months later, presented with reduced vision, diagnosed with bag subluaxion with phimosis, uveitis and secondary glaucoma right eye more then left. With medical management improved and were stable for 6 months. One year 3 months following surgery reported with sudden marked reduction in right eye vision, showed anterior dislocation of bag with IOL, which was removed and SFIOL was implanted. Unfortunately same thing happened in left eye 8 months letter, similar procedure was performed. RESULTS Following dislocated bag and IOL removal with SF IOL best corrected visual acuity in right was 6/12 and left eye was 6/9. CONCLUSION In- the-bag dislocation of IOL is a rare late complication in uveitis and scleral fixated intraocular lens is good options of restoring vision.
