cysticercosis is caused by larval form of taenia solium. intraocular infestation by cysticercous cellulose are more common compared to ocular adnexal involvement. Isolated orbital adnexal involvement is an uncommon occurrence in clinical practice. 24 year old male presented with complaint of painful swelling in left lower eyelid. on examination there was mass approximately of size 3×3 cm, with localised erythema and mild tenderness, firm in consistency, skin movable over the mass, transillumination test was negative. on ultrasonography a cystic lesion noted in lower lid of the left eye on the medial side measuring 1.2×0.9cm with hyper dense dot like structure within, suggestive of cysticercosis in the submuscular plane. patient serum IgM was strongly positive(>2.2).patient was started on oral albendazole and corticosteroid and was improved. after 2 weeks cyst was surgically removed and subjected to histopathological evaluation which established the diagnosis.
