Purpose:To report a case of natamycin resistant fungal keratitis caused by Aspergillus flavus species. Methods:Interventional case report describing the course and treatment of natamycin resistant Aspergillus flavus keratitis. Results: A 62-year-old gentleman presented to us with corneal ulcer in his left eye.The clinical examination suggested fungal keratitis. We did the corneal scraping and started natamycin 5% eye drops.The fungal ulcer responded well in first week but started worsening when we saw the patient after 10 days.The culture report revealed Aspergillus flavus species which was sensitive to voriconazole and fluconazole.On changing the treatment,there was both symptomatic and clinical improvement. Conclusion: Natamycin 5% is the first line of management of fungal keratitis and resistant to this drug is of major concern. Corneal scraping should be sent for culture and sensitivity in all keratitis patients as the culture report helped us in saving the eye of our patient.
