Abstract:
A 71year old male with history of recurrent masses in right eye since 1½year, which was painless and progressive in nature. HPE was OSSN. A non pigmented conjunctival mass extending 2mm into cornea in right eye. On slit lamp examination a conjunctival scarring suggestive of residual disease on temporal limbus extending upto 2mm into cornea with the mass showed keratin deposits on the surface. Rose Bengal stain showed stain positive. On clinical examination mass was suspected to have intraocular extension.Bscan showed a serous choroidal detachment nasally, non contiguous with the primary site. MRI showed orbital disease along lateral rectus and a serous choroidal detachment in opposite quadrant.Clinical disease was T4N0M0. Patient refused eye removal options. Orbitotomy with mass excision and separate scleral base excision done. Post operative Strontium 90 plaque brachytherapy given. The dilemmas in management of T4 OSSN with regards to eye preservation are highlighted.
