Purpose -To highlight importance of Imaging in Typical Optic neuritis(TON)
Introduction -Based on ONTT results for TON, demyelination was the most common cause. No need for laboratory (ESR, ANA) or radiological investigations (MRI) for diagnosing and treating TON was highlighted. It should be a clinical diagnosis.
Methodology -27 years old female, 4 months postpartum, presented with pain and defective vision RE for 2 weeks. BCVA RE was 6/60 with grade 3 RAPD. Ocular movements were painful. Fundus revealed RE disc edema, with defective central fields and colour vision. Clinically diagnosis of optic neuritis was made. Patient was adviced imaging in view of progressive vision loss for >10 days. CT chest and MRI Brain revealed miliary tuberculosis (TB).
Conclusion -Latent miliary and intracranial TB in an immunocompetent young person presenting as isolated ON has not been reported earlier. This case highlights importance of high index of clinical suspicion in present era of globalization.
