We present six patients in whom toric phakic IOL had been implanted for the correction of high myopic astigmatism with stable keratoconus. All patients had a history of contact lens intolerance, and refraction and corneal topography were stable for 2 years. Preoperatively, the highest and lowest manifest refraction was -12.00 D Sph -5.00 D Cyl x 130 in one case and -5.00 D Sph-3.75 D Cyl x 90 in other case. The surgery was uneventful and there was no postoperative complication. Postoperatively, uncorrected visual acuity and best spectacle-corrected visual acuity were markedly improved after implantation in all patients. No progressive sign of keratoconus was seen during 2-year follow-up. Our experience in 6 cases suggested that toric Phakic IOL implantation may be an alternative for the correction of high myopic astigmatism in eyes with stable keratoconus.
