Systemic Lupus Erythematosus (SLE) and Tuberculosis (TB) often presents with similar clinical features and can lead to diagnostic and treatment dilemmas. Here we report a 41 year old female who was diagnosed with SLE retinal vasculitis with vitreous hemorrhage. She tested positive for lupus anticoagulant. She was treated with pan retinal photocoagulation, pars plana vitrectomy and oral steroids. Eventually she developed neovascular glaucoma. She also had hemiplegia and dysarthria probably due to CNS Lupus. But the neurologist suspected CNS TB and started her on anti tubercular therapy which improved her neurological symptoms. Therefore, association of tuberculosis in systemic lupus erythematosus should always be considered.
