Rickettsia is one of the several causes for focal or multifocal retinitis post febrile illness and ocular involvement is thought to be an immune reaction to recent systemic Rickettsial infection. A 23yr old male presented with dimunition of vision from a week and a history of fever a month prior. BCVA was 2/60, fundoscopy showed focal haemorrhagic retinitis in the temporal quadrant and macular fan. Suspecting viral or Rickettsial etiology thorough investigations including vitreous tap for PCR was done. OCT macula showed intraretinal edema and subretinal fluid. Doxycycline, oral and topical steroids was started. 3 weeks later, BCVA had remarkably improved to 6/18, fundus showed resolving retinitis and macular edema. Weil felix test showed significant agglutination for OX-19 and OX-K. OCT macula showed intraretinal hyperreflectivity and a decrease in subretinal fuid. 6 months after the initial presentation the patient continues to maintain BCVA of 6/18.
